This policy explains how Project New Gen LLC, doing business as PJM Clinic (“PJM Clinic,” “we”), handles information collected through pjmclinic.com. PJM Clinic is opening soon; this website offers information and a waiting list.
Our website forms (waiting list, contact and insurance question forms) may collect: your name, phone number, email address (optional), preferred contact method, insurance type and plan or carrier name, type of visit, best time to reach you, the reason you are contacting us, and your consent choice. We do not ask for medical details. Please do not enter symptoms, diagnoses or other health information in our forms.
Form submissions are sent by email only to the clinic inbox, pjmclinic@pjmclinic.com, and are used to contact you about scheduling, the waiting list and clinic availability, and to answer your questions. We do not sell or rent your information, and we do not share it for marketing by others. We share information only with service providers that help us run the site and clinic, such as our email provider and our web host (Hostinger), and only so they can provide those services. We may disclose information if required by law, and if the practice is sold, merged, or transferred, information may be transferred to the new owner, who must use it consistently with this policy.
When you submit the waiting list form and check the consent box, you agree that PJM Clinic may contact you by your chosen method (phone call, text message or email) about scheduling and clinic availability. Consent to receive calls or texts is not a condition of receiving care or services. Message frequency varies. Message and data rates may apply. Reply STOP to any text to opt out and HELP for help, or call (623) 233-0651 or email pjmclinic@pjmclinic.com. We do not sell or share your mobile number or text-messaging consent with third parties or affiliates for their marketing purposes.
This website does not use analytics, advertising or tracking cookies. Our Contact and Home pages include an embedded Google Map and our Glendale page may too; when that map loads, Google may receive information such as your IP address and set its own cookies under the Google Privacy Policy. Our web host (Hostinger) keeps standard server logs, such as IP address, browser type and pages requested, for security and operation of the site.
This site does not track you across other websites or serve targeted ads, so browser “Do Not Track” or Global Privacy Control signals do not change how the site works. We would honor them if that ever changed.
Our site links to other websites, such as Google Maps and government resources. Those sites have their own privacy practices, and we are not responsible for them. Please review their policies before giving them information.
We keep waiting list information only as long as needed to contact you and schedule care, then delete it or move it into our scheduling system. To ask what information we have about you, or to have it corrected or deleted, email pjmclinic@pjmclinic.com or call (623) 233-0651.
This website is not directed to children under 13, and we do not knowingly collect information from children under 13 through it. Our forms should be completed by an adult 18 or older, or by a parent or guardian on a child’s behalf.
This is a website privacy policy. It is not PJM Clinic’s HIPAA Notice of Privacy Practices, which describes how we use and protect your health information as a patient; it is posted on this website and available at the clinic.
Our site uses HTTPS. No method of transmission over the internet is completely secure. We may update this policy; the effective date above shows the latest version.
